To all potential nursing staff We need the following documents from you before you can move forward in our on-boarding process:

Driver’s License/Photo ID/Passport (Color Copy)
SS Card (Color Copy)
LPN/RN Registration (Color Copy)
CPR Certification
Completed Physical within the past year (Statement of No Habituation OR Drug Screen must be included)
PPD/Chest X-Ray/Quantiferon Test results within the past year (If you have had a Chest X-Ray, results within the past 2 years)
Flu Vaccine (See Attached Document)

2 Letters of Reference from within the past 6 months
Voided Check
Passport picture for your employee ID Badge
